I'm installing a OBT laser to my router and after reviewing the schematic S15061, is it possible to share output 4 for laser enable and spindle forward? I'm using a rev. 3 board.

Not very likely that you can share the those terminals. Switching signals from 2 different voltage sources on the same terminals will cause problems. That said, you could use your OP4 to drive a small 4 pole relay that uses 2 contacts for the spindle, and 2 for the laser.
