"Z-Axis Communications Error 9016"

I've retrofitted a CNCNEW GT-27 mini lathe with an OAK CNC. I kept the original Mitsubishi Servo drives and the Yaskawa spindle drive.
I've changed the configuration from Z as channel 1 and X as channel 2, to X-axis as channel 1, and the Z=axis as channel 3. I've turned the home and EOT inputs off as this unit uses battery-backed encoders. I've tested the X, Z and Spindle encoders for feedback connection, and I see the counts change correctly.
I've solved all displayed errors except the 9016 error. The system seems ready to run. The emergency-stop output will not turn ON even though the E-stop input is functioning. This means the voltage to the spindle and servo motors is NOT on. (The estop output triggers the contactor ON)
The DF LED on the CNC board is NOT ON. I am using the ethernet cable supplied by Centroid. The 7-segment display shows a "1" but no flashing decimal-point. I am also using a Centroid-supplied and configured PC. I'm able to interface to any part of the CNC and I/O diagnostics.

Anyone know what ERROR 9016 is all about? :?:

Lathe operations will require Z as axis 1 and X as axis 2. Many of the built-in functions will not work correctly if you change them.

Please provide a report so we can see what you configuration and errors are. Most likely a error is stopping the estop release.

Good news! I restored the factory settings and moved the axis connectors to the correct points. The error is gone and the E-stop now enables main power to the drives and spindle. I have control of the spindle. (Hooray!) I now have all LEDs lit.
I've resolved the servo-amp E-stop issue as well. I think I am one answer short of operation.
