Took the motor off to check the connections. Connections were good but did find that the encoder coupler broke. Not sure why it broke. It is just an aluminum "flex" coupler and there shouldn't be any stress on the coupler. Luckily I had another one and installed it now everything seems smooth and working properly again. I did have to accept the new position for homing but now everything seems to be back to running perfect again

Hey, glad you found it. A probable reason a flex coupler is unaligned, be sure you're properly aligning the flex coupler when you're installing the new one, with no stress on it in the relaxed position.
