If you wired according to S15164 then input 5 should not be set to DriveOk. This should be removed from the Input Definitions in the wizard.

Ok, so we Removed DriveOk from input 5.
1) Then we Tested Axis 1 "X" which is also DMM1 of the Dyn4 Driver and upon powering up
everything the Green LED on DMM1 comes on upon Reset on the VCP.
2) When we try to Move X+ or X- we get an Message 9025 Axis 1 Drive Fault Detected.
Pressing Reset on the VCP does clear everything, but trying to move it again it Faults.
We Loaded DMMDRV4.exe and followed the instructions on page 32 of REV ZM5-A18A

After Selecting "Comset" and then Com Port 1
Select "Servo Setting" and select Dyn4
Press "Read" and after a couple of Seconds
DMMDRV4.exe closes instead of showing the On Screen Parameters as per the Instruction Manual?
Thus, the question for me is this a Acorn Problem or a DMM DYN4 problem and whom do I ask the
question or should I contact Centroid Technical support and just pay?
I have truly looked around and find very little detailing any of this other than the DMM Dyn4 manual
and the Schematic from Centroid for Connecting the Dyn4 which is how we wired things.

Is the drive faulting or just Acorn? More than likely you have the Axis1DriveOk configured incorrectly. NO when it should be NC or visa-versa. Or the drive configured incorrectly in the DMM software. I've only used the DYN5 drives so maybe someone who has configured the DYN4 drives can chime in.

You should also move the Axis1DriveOk input to Acorn and use input 1 thru 7. Use the additional inputs on the expansion card for less important inputs.

This is an Acorn6 and the Wiring Diagram S15164.r1 shows connecting
the Axis1 to the Input14 as seen in the attachment below. I am only following
what Centroid has provided. If it really should be connected to In1-7 kindly explain why?
We have the additional 2 plug and Play Relay 8 Modules and Ether1616 #14850 also.

When trying to Run the DMM Dyn4 Test like in the below, should the Dyn4 still be hooked up
to Acorn6 Control?

On page 3 of the above Referenced Manual it states that the DB9 Encoder Connector should Not
be Connected to the JP5 Port during Testing.
However, all of this Testing looks like it is done Only with the Dyn4 and AC Servo connected together,
but then I do not have control of the MC Magnetic Contactor to power on and off the power.
Can this Test be done with Acorn Connected?

I always recommend wiring any safety inputs, limits, e-stops, driveok, VFD faults etc to the Acorn directly because of latency on the external inputs on expansion cards. Just my 2¢
I've never used the DYN4 drives, they are "old school" now, only the DYN5 drives and they do not have the DB9 connector on them. I ran the DMMDRV5 software with the drives wired to a fully functioning machine. I just plugged in the mini-USB cable to each drive and launched the software on my laptop. I didn't disconnect anything from the drive or the Acorn. Again this was for a DYN5 drive.
