CNC10 Lost X/Y Axis Zeros (0,0) While Running Program

As title states, CNC10 machine with ATC was running a program that had successfully cut 30 pieces in the last few days, on the last part we ran T1 ran OK, but after tool change to T2, the program lost zero for X and Y axis. Subsequently the machine crashed the tool. There has been no modification to the program, it doesn't call an offset or different plane coordinate system.

The new X0/Y0 locations appeared to be almost dead center on the ways. Z axis did not appear to be affected, but we can't be sure.

Anyone ever experience anything like this?

The only explanation for this -other than a programmed or manual G52, G92 or WCS change, is a mechanical problem.

Please post or send a current report.zip from your system as well as the part program in question.
