Your control probably has a DC3IO drive/PLC unit powering the first three axes.
http://www.cncsnw.com/Drives.htm#DC3IO

You would need to add a DC1 drive; a DC servo motor; and appropriate cables.

If the DC servo motor you choose requires a lower voltage than the ca. 120VDC that your control supplies to the DC3IO for the existing motors, then you will need to add a suitable DC power supply for the new motor as well.

If the DC servo motor you choose can run on 120VDC, then you can just extend the existing 120VDC supply to the DC1.

Here is the Manual for DC-1:

Show pictures of your 4th axis.
What size motor (Frame and Power) do you need?

As previously mentioned, you need a DC-1, one of these:https://www.centroidcnc.com/rotary_tables.htm, and an internal bulkhead cable to connect the two.

I'd find my fourth axis first. Then find a DC brush servo, of which I have several.
