Your cncm.hom file doesn't have the home command for the 4th yet. Once you set the direction for the 4th, set "wizard to create" and then write the settings.

You can check the proxy by watching the input in the diagnostic screen while spinning the axis with the arrow keys.

I have attached the latest report and have change now to Wizard to generate automatic home program. The bolt is very close to the proxy, maybe 0.200" or less and when the bolt passes the proxy, the light on the proxy turns off and then back on again.
I have set the home for the A axis right on the bolt and proxy meet each other. But when I home the CNC (pressing PARK button) the A axis does not stop where the proxy meets the bolt, it bypasses it.
However I have more serious issue now (to me) because I am unable to home the CNC. I have a Home switch tripped and I am unable to home and use the cnc.
I have posted the latest report and PLC screenshot. HELP please :oops:

Jog the A axis until INP6 is green. Then press Cycle Start to find home.

Yeah! It works! Thank you!

Let me bring up another issue...
I am having somewhat of a loud pitch noise On the Z axis depending on what position is it on... Any ideas about what the issue might be and how can I troubleshoot it?

does the noise happen when the axis is moving or holding position?

and remind us what kind of axis motor and drive do you have?

a lot of axis motors will make noise when holding if there is play in the mechanical system or too tight! so look for either in the Z axis mechanism

Noise happens when the Z axis moves and even holding position, if I move a few thousands the noise goes away actually. I am using Clearpath motors.
I might do a video to give a better idea of what is going on.

ok does the noise come from the motors themselves? if so, sounds like you need to tune them.
a video of it would be great so we can hear what kind of noise it is.

Just recorded this with my GoPro. The noise stops when I move a few thousands of an inch up and down on the Z. I hope it makes sense about the noise I was complaining about, excuse the dust, this CNC is not a workbench queen 🤣 :

Problem solved. I little bit of grease to pump out any debris out the ball nut solved the problem. Learning as I go…
