I will double check inside the Wizard that I have the right state chosen for output 4 & 5.

Without the enable, that drive should not have turned on.

SUCCESS!!!

Third time is the charm. I had two bad KBMG-212D controller bought from ebay. The 3rd was new and works like a charm. This motor is very quite. Now I can move on with the rest of the restoration.

Thank you Marty for being so patient and helping me trouble shoot the problem. It was just bad luck (and a bit of frugality) that kept me from getting a good driver.
