I think that your assessment meshes with what John at Teknic said when I spoke with him this after noon.
I don't have twisted pair cables but have ordered them as well as resistors.
I'll be checking ALL connections and take fresh photos to share.
I am still not updated to version 4.5 trying to avoid any new variables while I modified the build and now 4.62 is available.
Should I load 4.5 before 4.62 or can I just update from an earlier version directly to 4.62 ?
John said I should run the oscilloscope in the clearpath MSP software to determine if the motor faulted first shutting it down causing the Z axis to fall to the hard stop or if there was a command which occurred first forcing the motor to run the Z axis down which after it hit the hard stop then recorded the drive fault.
I didn't realize that I was able to run MSP scope and Centroid CNC simultaneously .

I have more homework to do!

I would get upgraded to 4.62.
Take screen shots of all your current Wizard settings. Print them out.
Rename your current cncm directory to something like cncm - 4.50
Install a FRESH copy of 4.62.
Take the time using your screen shots to enter everything in manually. You'll be assured its a fresh install. Takes all of 15-30 minuts.

For Z axis I ordered the larger SDSK-3426P-ELN, the 3421's may have been adequate but went with the larger Z motor to have more mass to keep the spindle from falling rather than install a brake system or install a counter balance or gas spring system for the 26# spindle, dust boot, dust hose, mister, and laser. On the Z axis with the 2.54 turns per inch pitch, I am not sure you would need the ELN, but I spent the extra money just in case to make sure the system would perform how I wanted it to.

The Avid Pro Z Axis does have a drag brake on the ball screw to help keep the spindle from falling. I haven't had any issue with the spindle dropping with a drive fault or system powered off. However, the spindle did drop when I removed the motor to check the Oldham coupling. Again, this was more motor than I needed for the Z axis.

The 3411S-ELN would probably be adequate for your X & Y axis, I have way more power than I need with the motors I ordered, very pleased with the performance of the motors with the Acorn.

I only have one power supply and haven't had any issues with lack of power from the power supply for the 4 motors. I'm prepped for a 5th motor for a rotary axis.

My system is overbuilt and I spent more money than required on the Clearpath SDSK motors, but for me it was cheap insurance. I spent about $400 more than going with the smaller motors on X & Y and lower resolution motor for the Z axis. No Regrets.

And select a control cabinet a little larger than what you think you will need.
I just realized you said 5th Axis. Did you get a different controller? How are you making that work?

4 axis', 5 motors. The Y motors are slaved on one axis output with the Y-Slave motor direction inverted in the Clearpath motor settings. I have mechanically squared the gantry and am not utilizing the auto square feature of Acorn.

X-Y-Z-A Axis, A is be the rotary axis.

I just finished up my build, sdsk with hiwin ball screws and rails, 2 to 1 reduction on all axis, 5mm ball screw pitch.

HG30 bearings and rails (Hiwin)
R25-5T4 ball screw (w/5mm pitch) (Hiwin)

Here is a video of my mill running a facing operation, cutting speeds and feeds are as follows:
Cutting Parameters:
100 inches per minute
0.125" depth of cut
12,000 rpm
0.04" step over
~0.002 inches per tooth
