Here the picture, 24v is from power supply to com,, and oc3-oc8, i test by manual swicth spindle, reversed is ok oc5, got the light on relay module, and relay is respone,, but not working on oc4,, i change oc4 to oc3, and its work,

You did not post screen shots of your Wizard Input and Output Screens.
When you go into MDI and type this line
M3 S1000 and then press cycle start, does the output you assigned to Spinfwd turn on?
Then enter M5, press cycle start
Then enter M4 S1000, cycle start, does the output you assigned to SpinRev turn on?
Then enter M5, press cycle start
Right now its obvious you have nothing wired to the relay board. You show just the relay board but you don't show the where the other end of the wiring is connected. That picture is of not much help. You are making us assume you wired the other end correctly.
Do the above test and report back. Video clips of the test would be very helpful.

Yes marty,, i do the above test already,, m3s1000, and relay is not swicth on, is turn on when put m4 command is typed,, and then m5 is turn the relay off, but when i swicth oc4 to oc3 for spindle foward in the wizard, and is function after test with mdi, m3s1000 oc3 relay is on,,m5, ciyle start is turn of the relay,, i will try to make short video clip,, later,, thanks marty,,

The Input/Output screen shot shows that M3 should trigger output 3 and that M4 should trigger output 6

Yes and you have Output four set for Mist. M7 turns it on, M9 turns it off. You have to supply +24VDC to the relay board common. In your picture, you have a yellow wire going to it and then a terminal block, but you show nothing connected to the terminal block end. Its not going to work until you do....

That terminal block is a buss bar, i modify, 24vdc is on the leff, 0vdc is on the right,,24v coming from power supply an distribute for other uses,, and yes 24v relay board got the power, and i connect it to com terminal,,
