It is a valid G code file, describing a valid toolpath.
If it is not the toolpath you wanted, then that is for you to sort out with your CAD/CAM software. I cannot help you with that.

No, Centroid does not provide that capability in any obvious way. The usual solution, as you noted, is to have the PLC program send a signal back to the CNC macro, and do the servo axis movement in the CNC macro. You are correct that it would be technically better to use R1O, and configure it as a "drive ready" indicator.
The only reason that most Centroid installations use N.C. contacts and a "malfunction" status is 30 years of inertia.

I would guess it has a problem with the 200 inch distance you have entered for "Return Feed Amount". Try changing that to zero.
