H004 is for the number of poles not hZ. A003 & A004 are set according the rated hZ for the motor. My mistake
I noticed in your draft that A062 is set too 83.3. How did you come up with that number?

Wire the brake to one of the outputs and use a reverse bias diode to prevent any current from going back to the board. You also have to go into the wizard and setup the output for the axis that you are using the brake on, in your case Axis3BrakeRelease.

Did some more testing and found, that after homing, using the VCP jog buttons and MDI to jog the Y axis works fine. Seems to only happen when using the MPG right after homing.
