Please wire it per 15008r2. I'm not sure why there is a difference, but the wiring method in 15008 insures that only FWD or REV can be commanded at anytime. The wiring used in 14826 could allow both to be commanded at the same time. The PLC should prevent this but it could but the wiring in 15008 ...

Do you also have DCM jumpered to the center (common) terminals of your OUT4 and OUT5 relays?
The relay board has no internal connection between the commons of the different relays. They are all independent of each other.
I'm thinking that may be the problem, I didn't have DCM jumped to the ...

To be clear, DCM does not go to COM on H10 on the Acorn. It goes to the "commons" for the outputs you are using for forward reverse. Which outputs/relays on the relay board. Which relays is DCM wired to?
NOTE: The "common" on a relay is simply the voltage to be switched. It could be COM but it is ...
