Swissi, using your advice:
go into the Wizard -> DB25 Connector Mapping"
- turn on "Enable DB25 custom PIN mapping
- Exchange Step 3 with Step 4 and Dir 3 with Dir 4 ...

I wired it like the second schematic posted with no help. Just to see what would happen i enabled the 4th axis and connected step 4 to pul- and direction 4 to dir - on the driver and the axis worked. I'm assuming adding a report does not matter because with the 4.5 update it had stock settings and still had a problem?
