Where in the manual/instructions does it say what to do if you get this error?

First, are you testing with only your voltmeter leads connected to the Acorn's analog output? Do you have the max spindle speed set to 3000rpm per the instructions?
These things are 100% tested by Centroid before shipping so if something is wrong, it's usually a connection error. It's not a user adjustable item and probably means a trip back to Centroid if it really is wrong.
