I eliminated the possibility there is a wiring issue preventing my AcornSix board from driving XA, XB, and Y axes. I did this by replacing the AcornSix with an AcornCNC board. I plugged the H1 and H2 open collector connectors on the AcornSix directly into the H2 and H3 connectors on the AcornCNC. I reloaded CNC12V5.10 and entered the exact same configuration except for inputs, outputs, and homing. I manually homed the machine and gave it MDI commands. Cofigured with the AcornCNC, there are no issues with the paired XA and XB axes and no issues with the Y axis at either low or high speeds.

I will drop the board in the mail on Monday and provide a tracking number.
