9121 ATC Tool Counter noise

I have my tool counter set up using the geneva switch Normaly open, I can index radomly anywhere for 1 to 8 slots before I get the 9121 ATC tool counter noise error. Any help would be greatly appreciated.

make sure the signal wires to the sensor are shielded with a drain to earth ground.
