Make sure the machine chassis ground is connected to the power supply "COM" terminal.

1. No. This circuit removes the run signal from the VFD. 2. You could do that, but then you would want to wait a couple minutes for the VFD to completely power off every time you pressed Reset/E-stop. Does the enable LED for that axis turn green when you enter MDI or attempt to jog the axis?
