I'm hoping that the firmware is the problem. Now that I switched back from the AcornCNC to the AcornSix, only the XB axis provides movement.

Just to clarify a bit more. I used the open collector terminals for the XA, XB, and Y axes. I used the 5V step and direction terminals for the two Z axes.

I used the open collector terminals for the AcornSix. I used the same step rates for both boards.
The board is labeled PN, 15114, QC461 with a date of 3/15/23.

It worked perfectly for at least 2 years. I suspect that there was a homing failure as you say. I don't remember anything specifically. The report was made when I was in the process of diagnosing the problem.
I didn't have a problem with my set up. The machine worked fastastic for 3 years. Worked well after upgrading to V5.06.
One morning control of the Z axis with the VCP or WMPG-4 stopped.
